- Groups of three (3) or more must schedule an appointment.
- The Clerk's Office is only accepting passport photos from this location. No outside photos will be processed.

As of November 1, 2010 the Town of Islip will no longer consider a Puerto Rican birth certificate valid if it was issued before July 1, 2010.
The Puerto Rican government and the United States Department of State and Department of Homeland Security collaborated in creating a new law on the island to match documentation policies in the United States, according to the government of Puerto Rico's website. The birth certificate has become widely used to obtained fraudulent identification and documentation in the United States, so the federal agencies and territorial government made changes to how the certificates are issued.
Puerto Rican-born citizens living stateside can apply for a certificate at the website, www.pr.gov. Or you can write or call for a new one to: Department of Health, Demographic Registry, P.O. Box 11854, Fernandez Juncos Station, San Juan, PR 00910 (787) 767-9120 (English/Spanish speaking) or (787) 281-8868 (Spanish speaking only).
