To submit a Block Party Application, please fill out the online form below and attach all applicable forms.

  • BLOCK PARTY APPLICATION

*Cannot be submitted without attaching the Block Party Consent Form*
  • $75 fee via debit or credit.
  • Applicant MUST LIVE ON BLOCK WHERE PARTY IS TO TAKE PLACE.
  • Application MUST BE SUBMITTED 45 DAYS in advance of Block Party.
  • Requires 2 Signatures.
  • Must be Notarized.

    *Must be submitted with Block Party Application*
    • MUST provide names, addresses and signatures of approval from two-thirds (2/3) of homeowners on the block.
    • ONLY one signature per household will be accepted.
