Must attach the following forms:
  1. Valid Rabies Certificate (Required)
  2. Spayed/Neutered Certificate (If Applicable)

Includes Guide, Service, Police, Detection, Working, Search, Hearing, Deceased, Lost, dogs under 4 months old, and surrendered ownership.

  3. Exempt/Service Dog Certificate (Required for New Applications)
